The buck has to stop somewhere, though. Fault isn&#8217;t bad, per se. Something failed and an organization needs to find out why. If the organization is healthy, you can assign blame without people freaking out. Frank, honest conversations with other mature colleagues.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And, if nothing else, finding fault allows you to smoke test your operational systems. I see smoke pouring out with organizations all of the time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The initial wisps of smoke you see are usually around the feedback loop. People just don&#8217;t want to talk to each other.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ha! That always blew my mind. It&#8217;s amazing how many games you can play with a single deck of cards.
